Bug in Software?
Really annoying. Changes made in the editor do not populate / work on the site. I know it's not me, because an IISRESET command to the server makes everything start working again. Sometimes it works, and sometimes it doesn't - I haven't been able to replicate this "cache" problem with a repeatable pattern, it seems somewhat random as to whether it will work or not.

Hello,
Good news - it's not a bug. It's fixable.
The reason this happens is that ISAPI_Rewrite doesn't have permissions to read your config file. Resetting the pool makes the whole thing re-read all the config files on your server, that's why its the only thing that fixes it. To avoid IISRESET, you need to provide sufficient permissions to the config file(the folder it is located in) in accordance to this documentation - http://www.helicontech.com/isapi_rewrite/doc/security.htm

Hi there. I'm sorry, I don't think you read my entire post. Sorry, I'll be clearer:

There doesn't seem to be any pattern to whether or not it will save the changes correctly. Sometimes it will save them and sometimes it won't. After it stops working, only an IISRESET will work.
So, to recap, if this was indeed a permissions problem, it would never work properly.
I have seen the linked page that you posted before, and I already have verified that the permissions are correct. This error is still happening on the server.
I sense it may be happening when the file is saved within the same minute - it's almost like the server is not checking the updated timestamps correctly.
