I think it means "any character except a question mark". Is that correct?

Not sure whether a question mark inside a set of square brackets is treated as a literal or something else.

Yes, it means "any character except a question mark".
Actually, in symbolic classes only '-' symbol parsing as meta-symbol (but not always).
